ABSTRACT

SUMMARY: The preserved form of all components of the nerve fiber is a prerequisite for the proper conduction of the nerve impulse. various factors can change the shape of nerve fibers. In everyday practice, qualitative histological analysis is the gold standard for detecting changes in shape. Geometric morphometry is an innovative method that objectively enables the assessment of changes in nerve fibers' shape after local anesthetics action. A total of sixty sciatic nerves were used as material, which was intraneural injected with saline solution in the control group (n=30), and a solution of 1.33 % liposomal bupivacaine (n=30) in the test group. After the animals were sacrificed, nerve samples were taken and histological preparations were made. The preparations were first described and examined using a qualitative histological method, after which digital images were made. The images were entered into the MorphoJ program and processed using the method of geometric morphometry. Qualitative histological examination revealed no differences in nerve fibers after intraneurally applied physiological solution and liposomal bupivacaine. Using the method of geometric morphometry, a statistically significant change in the shape of axons was found after intraneurally applied saline solution and liposomal bupivacaine (p=0.0059). No significant differences in histological changes were found after the qualitative histological analysis of nerve fiber cross-section preparations. A statistically significant change in the shape of nerve fiber axons was observed after geometric morphometric analysis of digital images after intraneural application of saline and liposomal bupivacaine.


La forma conservada de todos los componentes de la fibra nerviosa es un requisito previo para la conducción correcta del impulso nervioso. Varios factores pueden cambiar la forma de las fibras nerviosas. En la práctica diaria, el análisis histológico cualitativo es el estándar de oro para detectar cambios de forma. La morfometría geométrica es un método innovador que permite evaluar objetivamente los cambios en la forma de las fibras nerviosas después de la acción de los anestésicos locales. Se utilizó como material un total de sesenta nervios ciáticos, que se inyectaron intraneuralmente con solución salina en el grupo control (n=30), y una solución de bupivacaína liposomal al 1,33 % (n=30) en el grupo de prueba. Después de sacrificados los animales, se tomaron muestras de nervios y se realizaron preparaciones histológicas. Primero se describieron y examinaron las preparaciones utilizando un método histológico cualitativo, después de lo cual se tomaron imágenes digitales. Las imágenes fueron ingresadas al programa MorphoJ y procesadas mediante el método de morfometría geométrica. El examen histológico cualitativo no reveló diferencias en las fibras nerviosas después de la aplicación intraneural de solución fisiológica y bupivacaína liposomal. Usando el método de morfometría geométrica, se encontró un cambio estadísticamente significativo en la forma de los axones después de la aplicación intraneural de solución salina y bupivacaína liposomal (p = 0,0059). No se encontraron diferencias significativas en los cambios histológicos después del análisis histológico cualitativo de las preparaciones de secciones transversales de fibras nerviosas. Se observó un cambio estadísticamente significativo en la forma de los axones de las fibras nerviosas después del análisis de morfometría geométrica de imágenes digitales después de la aplicación intraneural de solución salina y bupivacaína liposomal.

ABSTRACT

Introducción: a pesar de que un tercer molar no erupcionado repre- senta un riesgo de formación quística, la práctica clínica desestima el análisis histopatológico de los folículos de dichos molares. Objetivo: identificar la frecuencia de lesiones quísticas en los sacos pericoronarios de terceros molares mandibulares. Material y métodos: estudio des- criptivo, transversal, analítico y observacional, en donde se incluyeron sacos pericoronarios de terceros molares mandibulares para su análisis histopatológico, descripción de características clínico-radiográficas y su asociación con la presencia de cambios histológicos o lesiones quís- ticas. Resultados: se incluyeron 48 muestras de sacos pericoronarios, la histopatología de los sacos pericoronarios mostró que 83.3% tenían algún tipo de alteración: 13 quistes paradentales (27.1%), cuatro quistes dentígeros (8.3%), 12 folículos hiperplásicos (25.0%) y 11 folículos inflamados (22.9%). La presencia de lesiones quísticas en la población fue de 35.4%. Se detectó asociación estadísticamente significativa entre el sexo y la presencia de lesiones quísticas (p = 0.039) y entre el nivel de erupción y la presencia de cambios histológicos (p = 0.046). Con- clusiones: la frecuencia de lesiones quísticas o cambios histológicos en folículos de terceros molares mandibulares es alta, principalmente en molares parcialmente erupcionados o submucosos y sin importar la ausencia de sintomatología o alteraciones radiográficas (AU))


Introduction: although a non-erupted third molar represents a risk of cystic formation; clinical practice rejects the histopathological analysis of the follicles of said molars. Objective: identify the frequency of the histopathological changes in pericoronary sacs of mandibular third molars. Material and methods: descriptive cross- sectional, observational and analytic study, where pericoronary sacs of mandibular third molars were included for histopathological analysis, description of clinical-radiographic characteristics and their association with the presence of histological changes or cystic lesions. Results: 48 samples of pericoronary sacs were included, the histopathology of the pericoronary sacs showed 83.3% had some type of alteration: 13 paradental cysts (27.1%), four dentigerous cysts (8.3%), 12 hyperplastic follicles (25.0%) and 11 inflamed follicles (22.9%). The presence of cystic lesions in the population was 35.4%. A statistically significant association was detected between sex and the presence of cystic lesions (p = 0.039); and between the level of eruption and the presence of histological changes (p = 0.046). Conclusions: the frequency of cystic lesions or histological changes in mandibular third molar follicles is high, mainly in partially erupted or submucosal molars and regardless of the absence of symptoms or radiographic alterations (AU)

SUMMARY: Tissue clearing techniques are frequently used in the observation and description of anatomical structures and pathways without altering the three-dimensional layout of the anatomical specimen. Tissue optical clearing promotes preservation of three-dimensional structures, which allows the study of the internal anatomy in its original position and original spatial interaction. Among these techniques, Potassium Hydroxide (KOH) maceration clearing is one of the most widely used. However, the histological changes of tissue after KOH maceration have yet to be fully understood. Our aim is to describe the microscopical differences between macerated and normal tissue. To better understand said changes, two human fetuses with a gestation period of 16 to 28 weeks were cleared and processed for histological analysis. Microtome slides of the fetuses' lower limbs were obtained and stained with Hematoxylin & Eosin, Periodic Acid Schiff (PAS), and Masson's trichrome with the purpose of observing the histological and macromolecule composition changes in cleared tissue. Remarkable differences at a histological level regarding the composition of the cellular structures, since diaphanized tissues showed a predominance of extracellular matrix composed of collagen fibers with the absence of most of the nucleated cellular tissue. Phospholipid's saponification, nucleic acids degradation and a change on proteins structural properties are the main factors inducing clearing. At the same time, molecular stability of collagen in alkaline conditions allows the specimen to maintain its shape after the process.


RESUMEN: Las técnicas de limpieza de tejido se utilizan con frecuencia en la observación y descripción de estructuras y vías anatómicas sin alterar el diseño tridimensional de la muestra anatómica. El aclaramiento óptico de tejidos promueve la preservación de estructuras tridimensionales, lo que permite el estudio de la anatomía interna en su posición original y la interacción espacial original. Entre estas técnicas, el aclarado por maceración con Hidróxido de Potasio (KOH) es una de las más utilizadas. Sin embargo, los cambios histológicos del tejido después de la maceración con KOH aún no se han entendido por completo. Nuestro objetivo es describir las diferencias microscópicas entre el tejido macerado y el normal. Para entender mejor dichos cambios, dos fetos humanos con un período de gestación de 16 a 28 semanas fueron aclarados y procesados para análisis histológicos. Se obtuvieron microtomos de las extremidades inferiores de los fetos y se tiñeron con hematoxilina y eosina, ácido peryódico de Schiff (PAS) y tricrómico de Masson con el fin de observar los cambios histológicos y de composición de macromoléculas en el tejido aclarado. Diferencias notables a nivel histológico en cuanto a la composición de las estructuras celulares, ya que los tejidos diafanizados mostraban un predominio de matriz extracelular compuesta por fibras de colágeno con ausencia de la mayor parte del tejido celular nucleado. La saponificación de los fosfolípidos, la degradación de los ácidos nucleicos y un cambio en las propiedades estructurales de las proteínas son los principales factores que inducen la depuración. Al mismo tiempo, la estabilidad molecular del colágeno en condiciones alcalinas permite que la muestra mantenga su forma después del proceso.

Se desarrolló una investigación de innovación tecnológica con el objetivo de contribuir, a través de videos y enlaces a páginas con elementos teóricos, al desarrollo de habilidades prácticas sobre técnicas histológicas básicas que incluye el procesamiento de los tejidos para ser observados en el microscopio óptico; así como la tinción de éstos con Hematoxilina y eosina. Para su diseño se empleó una plantilla web (boostrap, html5, CCS). En cuanto a los aspectos metodológicos: se tuvo en cuenta el programa de formación de residentes en la especialidad de Histología y de pregrado de la asignatura de Células, tejidos y tegumentario, propuesto por la Universidad de Ciencias Médicas de La Habana (UCM H). Se validó el producto a través del criterio de expertos y de usuarios; obteniéndose un elevado nivel de aceptación; permitiendo el desarrollo de habilidades prácticas, de forma amena y científica en los estudiantes y residentes de la especialidad de Histología(AU)


A technological innovation research was developed with the aim of contributing, through videos and links to pages with theoretical elements, to the development of practical skills on basic histological techniques that includes the processing of tissues to be observed in the optical microscope; as well as staining of these with Hematoxylin and eosin. For its design a web template (boostrap, html5, CCS) was used. The methodological aspects taking into account were: the training program for residents in the specialty of Histology and undergraduate program of the Cells, Tissues and Integumentary subject, proposed by the UCM H. The product was validated through the criteria of experts and users; obtaining a high level of acceptance; allowing the development of practical skills, in an entertaining and scientific way in students and residents of the specialty of Histology(AU)

ABSTRACT

El molusco contagioso es una patología viral benigna muy frecuente, exclusiva del ser humano, y causada por un virus no clasificado del grupo de los Poxvirus. Las manifestaciones clínicas de la enfermedad inclu-yen lesiones en la piel, que pueden variar desde una pequeña pápula a un nódulo de mayor tamaño, pre-sentándose en forma solitaria o múltiple, dependien-do del estado inmunitario del paciente y del tiempo de evolución del proceso morboso. El estudio histo-patológico es importante para el diagnóstico, aunque en numerosas ocasiones éste se define clínicamen-te. Además del patrón histológico tradicional, y más frecuente, que exhibe hiperplasia e hipertrofia de la epidermis, se han descripto variantes poco usuales, cuyas características dependen, entre otros factores, de la sobreinfección y de la respuesta inmunitaria del paciente. En este trabajo se describen los rasgos ge-nerales del molusco contagioso y luego se presentan varios casos clínicos, uno de los cuales exhibe ma-nifestación inusual en la semimucosa del labio. Por último, se realizan comentarios referentes a la im-portancia que tiene para el odontólogo conocer esta patología y estar capacitado para detectarla, de modo de evitar sus complicaciones y su diseminación (AU)


Molluscum contagiosum is a very common benign viral pathologythat affects exclusively humans and is caused by an unclassified virus of the Poxvirus family. Clinical manifestations include skin lesions such as papule or nodule, which may range from a small papule to a larger nodule, presenting either solitary or multiple, depending on the immune status of the patient and the time of evolution of the morbid process. Histopathological study is important for the diagnosis, although in numerous occasions it is defined clinically. Classical and more frequent histology pattern exhibits hyperplasia and hypertrophy of the epidermis; however, distinct characteristics may occur depending on factors like superinfection and immune response of patients. This article describes general aspects of molluscum contagiosum and exposes several clinical cases, one of which exhibits an unusual manifestation in the semimucosa of the lip. Finally, comments are made regarding the importance for dentists to learn about the existence of this pathology and be able to recognize it in order to avoid its complications and spread (AU)

SUMMARY: In surgical and anatomical training, use of cadaver remains the most ideal technique. Standard formaldehyde solution preserves cadaveric tissues for an extended period comparing to the unfixed tissues. However, it fails to retain the natural texture, color, and biomechanical features. Phenol based soft embalming methods were developed to maintain these properties, while simultaneously decreasing the biohazard risk. Soft embalming techniques have made the bodies more 'lifelike' and wellfitted for training. Though phenol fixation displays rewarding morphological maintenance, we have scanty evidences on the histological preservation. This mini review primarily discussed the latest reports regarding the effect of phenol-based fixation on the tissue histology. Published literatures revealed phenol-based fixation displayed comparable histological preservation to that ofgold standard paraformaldehyde-based solution. It was concluded that phenol-based solution is an excellent fixative used to preserve tissues for microscopic analysis.


RESUMEN: En el entrenamiento quirúrgico y anatómico, el uso de cadáveres sigue siendo la técnica más ideal. La solución estándar de formaldehído conserva los tejidos cadavéricos durante un período prolongado en comparación con los tejidos no fijados. Sin embargo, no conserva la textura, el color y las características biomecánicas naturales. Se desarrollaron métodos de embalsamamiento blando a base de fenol para mantener estas propiedades y, al mismo tiempo, disminuir el riesgo biológico. Las técnicas de embalsamamiento suaves han hecho que los cuerpos sean más "realistas" y estén mejor preparados para la enseñanza. A pesar que la fijación de fenol muestra un buen mantenimiento morfológico, existe evidencia escasa sobre la preservación histológica. Esta mini revisión se refirió principalmente a los últimos informes sobre el efecto de la fijación en base de fenol en la histología del tejido. La literatura publicada reveló que la fijación a base de fenol mostró una preservación histológica comparable a la de la solución a base de paraformaldehído. Se concluyó que la solución a base de fenol es un excelente fijador utilizado para preservar tejidos para análisis microscópico.

ABSTRACT

Microscopic findings in key tissues are often critical to determine the cause of death in medical autopsies. The overall quality of histologic sections depends on numerous pre-analytic factors, among which are tissue section size and thickness. We designed a prospective quality improvement study to determine whether a simple intervention of formalin pre-fixation of myocardium, liver, and kidney tissues could improve the ease of cutting and quality of autopsy histologic sections as assessed by histotechnicians and pathologists. Of 46 autopsies included in the study, 21 were randomly assigned to formalin pre-fixation, and 25 underwent routine sectioning without formalin pre-fixation. A significant improvement in overall quality score by histotechnicians was detected in the sections from pre-fixed autopsy tissues compared to the control group (p=0.0327). There was no significant difference in quality score between the two groups as assessed by pathologists. Our autopsy quality improvement study demonstrates that a simple, low-cost intervention of formalin pre-fixation of fresh autopsy tissues for 90 minutes could significantly improve the overall quality of sections submitted for histologic processing.

Objective: To compare salivary transferrin levels between patients with oral lichen planus (OLP) and healthy subjects. Material and Methods: In this descriptive, analytical, crosssectional study, 11 patients with OLP and 22 healthy subjects were selected after matching in terms of age and gender. OLP was confirmed by two oral medicine specialists based on clinical and histopathological criteria. Salivary samples were collected by spitting. The patients were asked to collect their saliva in their oral cavity and then evacuate it into sterilized Falcon tubes. The procedure was repeated every 60 seconds for 5-15 minutes. A total of 5 mL of saliva was collected using this method. The samples were collected from 8 to 9 in the morning in a fasting state to avoid circadian changes. The collected salivary samples were immediately placed next to ice and transferred to the laboratory to be centrifuged at 4°C at 800 g to isolate squamous cells and cellular debris. Then the samples were frozen at -80°C until the samples were prepared. An ELISA kit was used to determine salivary transferrin levels. Data were analyzed with descriptive statistics (means and standard deviations) and t-test for independent groups using SPSS 17. Statistical significance was set at p<0.05. Results: The mean salivary transferrin concentrations in patients with OLP and healthy subjects were 0.9055±0.28229 and 1.5932±0.80041 mg/dL, respectively (p<0.05). Conclusion: The salivary transferrin levels in patients with OLP were significantly lower than those in healthy subjects.

Abstract: Melanoma Guidelines of the Brazilian Dermatology Society recommend histologic review by pathologists trained in melanocytic lesions whenever possible. Out of 145 melanoma cases identified at a private clinic in São Paulo/Brazil, 31 that had been submited to histologic review were studied to evaluate whether revision had led to change in therapeutic approach.. Differences in original/reviewed reports were found in 58.1% (n=18) of the reports, leading to changes in therapeutic approach in 41.9% (n=13). Change in diagnosis was observed in 6 out of 31 (19,3%) cases. These findings suggest that second opinion by pathologists trained in melanocytic lesions is likely to show significant differences from the original report.

Os autores descrevem as alterações histológicas no músculo grande dorsal submetido à expansão após relaxamento com toxina botulínica e as possíveis correlações dos achados com os benefícios práticos como, por exemplo, aumento da complacência muscular e melhor acomodação da prótese. Foi empregado o modelo experimental, com dez ratas com peso médio de 300 g, mesma faixa etária, da cepa Wistar (Rattus norvegicus) e o músculo grande dorsal. Biópsias musculares foram feitas antes e após as expansões, no músculo normal, no grupo controle (apenas com expansores) e no grupo com expansores e toxina botulínica. Expansores de 3 centímetros cúbicos eram posicionados abaixo do músculo e expandidos com 0,3 mililitros de soro fisiológico semanalmente, por 10 semanas. Os cortes histológicos foram corados segundo as técnicas de Hematoxilina-eosina, para avaliação geral, e tricrômio de Masson para avaliação do tecido conjuntivo. As fibras musculares submetidas à expansão sob a ação da toxina botulínica apresentaram focos de fibrose e proliferação de vasos sanguíneos menos intensos que no grupo sem toxina botulínica e a diminuição do número de fibras musculares e a atrofia eram menores que no grupo que não utilizou a toxina. Os achados nos permitem presumir que a expansão muscular associada ao relaxamento com toxina botulínica preserva as características da musculatura esquelética, oferecendo melhor acomodação e proteção da prótese e facilitando a dinâmica da expansão, além de diminuir a dor.


The authors describe histological changes in the latissimus dorsi muscle submitted to expansion after relaxation with botulinum toxin. The possible practical benefits include increased muscle compliance and better accommodation of a prosthesis. The experimental model involved 10 Wistar rats (Rattus norvegicus) of the same age, with average weight of 300 g. Muscle biopsies before and after expansion were performed in normal muscle, in a control group (with expanders alone), and in a group with expanders and botulinum toxin. Expanders measuring 3 cm3 were positioned below the muscle and expanded with 0.3 ml of saline weekly, for 10 weeks. Histological sections were stained using hematoxylin-eosin for general evaluation and Masson's trichrome for evaluation of connective tissue. The muscle fibers submitted to expansion under the action of botulinum toxin showed less fibrosis and less intense proliferation of blood vessels than in the group without botulinum toxin, and the atrophy and reduction in the number of muscle fibers were less prominent than in the group that did not receive botulinum toxin. The findings suggest that muscle expansion associated with botulinum toxin relaxation preserves skeletal muscle characteristics by providing better accommodation and protection for a prosthesis and facilitating expansion dynamics; this method may also reduce pain.

Introducción: el endometrio es fundamental para el diagnóstico de entidades que constituyen problemas de salud para la población femenina. Resulta de vital importancia para el patólogo el hallazgo de diferencias entre la morfología del endometrio de la hiperplasia endometrial y el adenocarcinoma endometrioide que le permitan realizar el diagnóstico diferencial ente estas dos entidades. Objetivo: determinar las diferencias morfométricas para el diagnóstico histopatológico diferencial entre la hiperplasia endometrial compleja, el adenocarcinoma endometrioide y en el endometrio proliferativo normal. Materiales y métodos: se realizó un estudio observacional descriptivo de corte transversal y se aplicó morfometría a una muestra de 30 biopsias endometriales, con el objetivo de determinar las diferencias morfométricas para el diagnóstico histopatológico diferencial entre la hiperplasia endometrial compleja, el adenocarcinoma endometrioide y en el endometrio proliferativo normal. Se utilizó sistema morfométrico IMAGEN J 1.44p, se estudiaron las glándulas endometriales a las que se les halló la altura del epitelio glandular. Resultados: la altura del epitelio en las glándulas de tamaño menor, intermedio y mayor, expresaron estrechas diferencias entre la hiperplasia endometrial compleja, el adenocarcinoma endometrioide y en el endometrio proliferativo normal y que existen diferencias entre la hiperplasia endometrial y el adenocarcinoma endometrioide cuando se estudian variables que tiene en cuenta la morfología glandular. Conclusiones: se concluyó que existen diferencias entre la hiperplasia endometrial y el adenocarcinoma endometrioide cuando se estudian variables que tiene en cuenta la morfología glandular (AU).


Introduction: the endometrium is fundamental for the diagnosis of entities that are a health problem for female population. It is very important for the pathologist to find differences between the morphology of the endometrium of the endometrial hyperplasia and the endometrial adenocarcinoma allowing to perform the differential diagnosis between these two entities. Objective: to determine the morphometric differences for the differential histopathological diagnosis between the complex endometrial hyperplasia, the endometrial adenocarcinoma and the normal proliferative endometrium. Materials and methods: a cross-sectional, descriptive, observational study was applied and the morphometry was applied to a sample of 30 endometrial biopsies, with the objective of determining the morphometric differences for the histopathological differential diagnosis among complex endometrial hyperplasia, endometrial adenocarcinoma and normal proliferative endometrium. The morphometric system IMAGEN J 1.44p was used and the endometrial glands were studied and the height of their glandular epithelium was calculated. Results: the minor, intermediate and higher height of the epithelium in the glands expressed tight differences among complex endometrial hyperplasia, endometrial adenocarcinoma and normal proliferative endometrium, and showed that there are differences between endometrial hyperplasia and endometrial adenocarcinoma when the studied variables take into account the glandular morphology. Conclusions: there are differences between endometrial hyperplasia and endometrial adenocarcinoma when there are studied variables taking into account the glandular morphology (AU).

ABSTRACT

Introducción: el granuloma telangiectásico es un tumor reactivo inflamatorio en la cavidad bucal a causa de factores irritativos, traumafísico o factores hormonales. Es más frecuente en el sexo femenino entre la segunda y sexta décadas de la vida. Objetivo: describir los hallazgos clínico-patológicos del granuloma telangiectásico bucal, su posible patogénesis, diagnóstico diferencial y manejo terapéutico. Presentación de casos: se obtuvieron los datos de edad, género y diagnóstico clínico de una serie de cinco casos de granuloma telangiectásico bucal diagnosticados en la Facultad de Odontología de la Universidad de Cartagena entre el año 2016 y 2017. Se describen características clínicas, histológicas, factores etiológicos y sintomatología presentada en cada uno de ellos. El diagnóstico se realizó mediante histopatología y se confirmó como granuloma telangiectásico. En la totalidad de los casos el tratamiento realizado fue netamente quirúrgico. Conclusiones: es importante que los profesionales en odontología conozcan todos y cada uno de los aspectos relacionados con el granuloma telangiectásico, características clínicas, histológicas y los factores etiológicos, con el objeto de poder ofrecer un diagnóstico certero y, por consiguiente, un adecuado plan de tratamiento(AU)


Introduction: telangiectatic granuloma is an inflammatory reactive tumor of the oral cavity caused by irritating agents, physical trauma or hormonal factors. It is more frequent in the female sex between the second and sixth decades of life. Objective: describe the clinical-pathological findings of oral telangiectatic granuloma, as well as its possible pathogenesis, differential diagnosis and therapeutic management. Case presentation: data were obtained about the age, sex and clinical diagnosis of a series of five cases of oral telangiectatic granuloma diagnosed at the School of Dentistry of the University of Cartagena in the period 2016-2017. A description is provided of clinical and histological characteristics, etiological factors and symptoms of each case. Histopathology confirmed the diagnosis of telangiectatic granuloma. In all cases treatment was exclusively surgical. Conclusions: it is important for dental professionals to be aware of every single detail concerning telangiectatic granuloma, such as its clinical and histological characteristics and etiological factors, to be able to reach an accurate diagnosis and apply the appropriate therapy(AU)

Se realizó un estudio analítico y comparativo de 553 biopsias realizadas en el Hospital General Docente Dr Juan Bruno Zayas Alfonso de Santiago de Cuba en el período 2014-2015, con vistas a valorar la utilidad de las coloraciones histoquímicas de mucina: PAS, azul alcian (2,5 pH) y PAS-azul alcian, para diferenciar las lesiones benignas y malignas de la próstata. En la serie se obtuvo 98,4 por ciento de hiperplasias benignas positivas con el PAS y ninguna de ellas con el azul alcian, mientras que 50,9 por ciento de todos los carcinomas resultaron positivos con esta última; por el contrario, para los carcinomas bien diferenciados, la positividad con el azul alcian resultó ser de 96,3 por ciento y solo de 3,6 por ciento con el PAS. Se concluye que las coloraciones histoquímicas de mucina pueden ser usadas como herramientas para distinguir el adenocarcinoma bien diferenciado de la hiperplasia prostática


A comparative analytical study of 553 biopsies was carried out in "Dr. Juan Bruno Zayas Alfonso" Teaching General Hospital in Santiago de Cuba during 2014-2015, aimed at valuating the usefulness of mucin histochemistry stains: PAS, alcian blue (2.5 pH) and PAS-alcian blue, to differentiate the prostate benign and malign lesions. In the series, 98.4 percent of positive benign hyperplasias with PAS was obtained and none of them with alcian blue, while 50.9 percen of all carcinomas were positive with the latter; on the contrary, for the well differentiated carcinomas, the positivity with alcian blue was 96.3 percen and just 3.6 percen with PAS. It was concluded that mucin histochemistry stains can be used as tools to distinguish the well differentiated adenocarcinoma from the prostatic hyperplasia

Introdução: A manobra de liberação dérmica periareolar na mamoplastia promove uma melhor mobilidade do complexo mamilo-aréola (NAC). No entanto, existem dúvidas sobre possíveis danos nos nervos neste tipo de topografia. Este estudo objetiva uma análise quantitativa comparando a densidade dos nervos do fluxo lateral medial, lateral e caudal do NAC. Métodos: Trata-se de um estudo prospectivo. O estudo incluiu 26 pacientes do sexo feminino que foram submetidas à mamoplastia redutora pela técnica clássica de Pitanguy. Os fragmentos da derme coletados dos lados medial, lateral e caudal foram devidamente preparados e submetidos a um estudo histológico para determinar a densidade dos nervos em cada um dos lados estudados. Resultados: Dos 26 pacientes estudados, 42,3% apresentaram maior densidade de nervos no lado lateral; 38,5%, do lado medial e 19,2% do lado caudal. A análise estatística utilizada para avaliar se houve predominância de um lado onde a derme foi seccionada demonstrou que o teste de comparação de proporções não foi significativo (p = 0,304). Conclusão: A análise comparativa mostrou que não há preponderância de densidade de nervos em qualquer lado da derme periareolar.


Introduction: The periareolar dermal release maneuver in mammoplasty promotes better mobility of the nipple-areola complex. However, there are doubts on possible nerve damages in this kind of topography. This quantitative analysis compared the nerve branches density from the medial, lateral and caudal side-flow of the nipple-areola complex (NAC). Methods: This was a prospective study. The study included 26 women who have undergone a mammaplasty reduction using the Pitanguy's classic technique. The dermis fragments collected from the medial, lateral and caudal sides were properly prepared and subjected to a histological study in order to determine the nerve branches density in each studied sides. Results: Of 26 studied patients, 42.3% had a higher nerve branches density in the lateral side; 38.5%, on the medial side and 19.2% on the caudal side. The statistical analysis used to evaluate whether there was a predominance of one side where the dermis has been sectioned showed that the proportion comparison test was not significant (p = 0.304). Conclusion: The comparative analysis has shown that there is no preponderance of nerve density in any periareolar dermis side.

Fundamento: Alta ingestão de lipídeos e colesterol compõe a dieta da sociedade moderna e está envolvida com o desenvolvimento de doenças cardiometabólicas. Entretanto, há lacunas na literatura quanto à existência de modelos de dislipidemias em ratos Wistar. Objetivos: Analisar o perfil cardiometabólico de ratos Wistar alimentados com dieta hiperlipídica e hipercolesterolêmica por seis semanas. Métodos: Ratos Wistar jovens foram alimentados com dieta hiperlipídica e hipercolesterolêmica por seis semanas para induzir a hiperlipidemia. Os ratos foram submetidos à cateterização da artéria carótida para determinar a pressão arterial. Após jejum, amostras de sangue foram coletadas por meio do cateter, e as concentrações de colesterol total, colesterol HDL, triglicerídeos e glicose foram determinadas. Amostras do tecido cardíaco foram removidas para análise histológica, a fim de se verificar a hipertrofia ventricular. Resultados: A ingestão da dieta por seis semanas foi eficaz em induzir alterações cardiometabólicas. O perfil dislipidêmico apresentado pelos ratos Wistar foi acompanhado de hiperinsulinemia, hipertensão moderada e hipertrofia ventricular do coração. Não houve alterações na glicemia. Conclusões: A administração de dieta hiperlipídica e hipercolesterolêmica em ratos Wistar jovens por seis semanas induziram alterações cardiometabólicas tornando-se um modelo eficaz para distúrbios dessa natureza


Background: The diet of modern society is composed by large intakes of lipids and cholesterol, involved in the development of cardiometabolic diseases. However, there are gaps in the literature regarding the existence of dyslipidemia models in Wistar rats. Objectives: To analyze the cardiometabolic profile of Wistar rats on a six-week hyperlipidic, hypercholesterolemic diet. Methods: Young Wistar rats were kept on a hyperlipidic, hypercholesterolemic diet for six weeks to induce hyperlipidemia. The rats underwent catheterization of the carotid artery to determine blood pressure. After fasting, blood samples were drawn through the catheter, and concentrations of total cholesterol, HDL cholesterol, triglycerides and glucose were determined. Cardiac tissue samples were taken for a histological analysis to check for ventricular hypertrophy. Results: The six-week diet was effective in inducing cardiometabolic alterations. The dyslipidemic profile presented by the Wistar rats was accompanied by hyperinsulinemia, moderate hypertension and cardiac ventricular hypertrophy. There were no alterations in glycemia. Conclusion: The six-week hyperlipidic, hypercholesterolemic diet in young Wistar rats induced cardiometabolic alterations, becoming an effective model for disorders of this nature

O infarto agudo do miocárdio (IAM) é uma situação clínica determinada por processo isquêmicoagudo, que resulta em necrose miocárdica. Os marcadores cardíacos em caso de isquemia reversível, atualmente,apresentam sensibilidade limitada.Objetivo: Verificar a sensibilidade da albumina modificada isquêmica (AMI), como marcador cardíaco. Métodos: Estudo experimental, realizado no Laboratório de Experimentação Animal da Universidade Regional Integrada (URI), Erechim, RS, no período de 2011 a 2013. Após a indução isquêmica do miocárdio em ratos da linhagem Wistar-Tecpar, com idade aproximada entre 60-90 dias, através da administração de isoproterenol hidrocloridrato, o conteúdo da AMI foi avaliado em diferentes tempos. Background: Acute myocardial infarction (AMI) is a condition determined by an acute ischemic process resulting in myocardial necrosis. Cardiac markers in reversible ischemia currently have limited sensitivity. Objective: To check the sensitivity of ischemia modified albumin (IMA) as a cardiac marker.Methods: Experimental study held at the Animal Experimentation Laboratory of Universidade Regional Integrada (URI), Erechim, RS, from 2011 to 2013. After myocardial ischemic induction in Wistar-Tecpar rats aged about 60-90 days through administration of isoproterenol hydrochloride, the IMA content was evaluated at different times. Introducción: en la actualidad, la principal estrategia para controlar la infección por Mycobacterium leprae es la detección precoz y la multiterapia. El objetivo del presente estudio fue evaluar la utilidad del empleo de diferentes muestras clínicas en el diagnóstico molecular de la infección por M. leprae. Métodos: se evaluaron 22 pacientes con clínica sugestiva de lepra. Se tomaron muestras de linfa de cuatro puntos (ambos lóbulos auriculares y ambos codos) recogidas en una lámina porta objetos (lámina de baciloscopía) y en un hisopo, además muestra de hisopado nasal, muestra de sangre total y de tejido de la lesión. Las muestras se analizaron mediante baciloscopía, histología y PCR según correspondió en cada caso. Por otro lado, para determinar la sensibilidad y especificidad del método, se realizó un estudio de casos y controles en el que se emplearon 40 láminas de baciloscopías negativas de pacientes con lepra paucibacilar y 40 láminas negativas de personas sin lepra. Resultados: el 54,5 por ciento de los pacientes resultó positivo por baciloscopía. Solo en el 41 por ciento de los pacientes la histología tuvo resultados concluyentes de lepra. En el 100 por ciento de los pacientes se detectó la presencia de ADN de M. leprae a partir de la lámina de baciloscopía y el hisopado de linfa. En el 95,45 por ciento de los pacientes se pudo amplificar la secuencia diana a partir de la sangre total y solo en el 31,8 por ciento de los pacientes el hisopado nasal resultó positivo. El estudio de casos y controles mostró que la sensibilidad y especificidad de la PCR respecto al diagnóstico convencional fue de 100 por ciento. Conclusión: el diagnóstico de M. leprae mediante PCR, es de gran utilidad cuando las técnicas convencionales no son concluyentes. La lámina de baciloscopía y el hisopado de linfa constituyen las muestras clínicas más útiles para la confirmación molecular de la infección por M. leprae(AU)


Introduction: Currently, the early detection and treatment with multitherapy are the main strategy to control the infection by Mycobacterium leprae. The objective of the present study was to evaluate the usefulness of different clinical samples for molecular diagnosis of M. leprae infection. Methods: Twenty two patients with suggestive clinical leprosy were analyzed. Different clinical samples were taken by slit skin smear, histopathology and PCR. To determine the sensitivity and specificity of the PCR method, a case-control study was also performed using 40 slides from negative smears of patients with leprosy paucibacillary and 40 from individuals without leprosy. Results: From all patient studied fifty-four percent were positive by slit skin smear and 41 percent were conclusive of leprosy by histopathology. M. leprae DNA was detected in slit skin smears and lymph swabs in 100 percent of patients. In 95,45 percent of patients were detected M. leprae DNA in whole blood and in 31,8 percent of them in nasal swab. The sensitivity of PCR respect to conventional diagnostic was 100 percent, the specificity was 97.5 percent, and the positive predictive value was 97.56 and the negative predictive value was100 percent. Conclusion: The diagnosis of M. leprae by PCR is very useful when conventional techniques are inconclusive. The slit skin smears and lymph swabs are the most useful clinical samples for molecular confirmation of infection with M. leprae(AU)

ABSTRACT

La diafanización dentaria consiste en lograr transparentar la estructura mineralizada, permitiendo observar la arquitectura interna del diente. Se realiza principalmente a partir de métodos por deshidratación de la muestra. Por otro lado existen técnicas utilizadas para el estudio de centros de osificación basadas en métodos por maceración utilizando hidróxido de potasio y glicerina, de las cuales no se tiene antecedentes de haber sido utilizada en diafanización dentaria. El objetivo de este trabajo fue describir una técnica de diafanizado por maceración de tejidos dentarios mediante hidróxido de potasio y mostrar el resultado obtenido con un seguimiento de 5 años. Se seleccionó una muestra de 40 dientes (10 incisivos, 6 caninos, 10 premolares y 14 molares). Se limpiaron los dientes (destartraje) y luego se descalcificaron parcialmente con ácido clorhídrico. El 25% de la muestra fue diafanizada con el método clásico de deshidratación. El 75% restante fue diafanizado con el método por maceración, que consistió básicamente en una maceración parcial del diente mediante baños en hidróxido de potasio de baja concentración, luego agregando glicerina en forma ascendente hasta terminar en glicerina pura por tiempo indefinido. La técnica por maceración logró diafanización adecuada de los tejidos con una óptima visualización del sistema de canales radiculares en las muestras diafanizadas. Con ambas técnicas se logró transparentar los dientes pero con diferencias evidentes. Con la primera los dientes lograron un tono amarillento, perdiéndose gran cantidad de tejido dentario en cambio las muestras diafanizadas por maceración conservaron mejor su estructura y obtuvieron un color más cercano al del diente original. Con el método por maceración se obtienen dientes diafanizados más similares a la realidad que lo logrado con el método por deshidratación.


Dental clearing objective is performed to obtain the translucency of the mineralized structure, allowing an appreciation of the inner architecture of the tooth. It is achieved mainly using the dehydration technique on the sample. On the other hand, there are other techniques used for the study of ossification centers based on the maceration methods using potassium hydroxide and glycerin, none of which have been reported for dental clearing. The purpose of this study was to describe the maceration technique of the dental tissue using potassium hydroxide with a five-year follow up. We selected 40 teeth to complete the sample (10 incisors, 6 cuspid, 10 bicuspids and 14 molars). The teeth were cleaned and then partially decalcified with hydrochloric acid. A quarter of the sample was cleared by classic dehydration method. The other three quarters were cleared with the maceration method, that consisted mainly in a partial maceration using potassium hydroxide baths at low concentrations, followed by the addition of glycerin at ascending concentrations and finally using pure glycerin for an undefined period of time. The maceration technique achieved an adequate clearing of the mineralized tissue with an optimal visualization of the root canal system in the cleared sample. With both techniques it was possibly to clear the teeth but with evident differences. With the first technique the teeth achieved a yellowish color, losing a great amount of dental tissue however the maceration technique sample allowed the preservation of more structure and obtained a coloration similar to the original tooth. With the maceration method the samples were more similar to the original ones than the cleared teeth obtained with dehydration.

ABSTRACT

La resina poliéster (P-4) se ha utilizado en varias técnicas anatómicas, gracias a su bajo costo, fácil manejo y obtención. Entre ellas la inclusión, que se realiza en capas dejando muestras relativamente opacas y de un grosor que no permite ver claramente las estructuras. También encontramos la plastinación de cortes anatómicos, técnica más sofisticada, compleja y de un alto costo económico. El objetivo fue idear un método de inclusión para cortes anatómicos, que mejore la calidad de la técnica clásica. Se utilizaron cortes anatómicos de segmento distal de miembro inferior humano y de un grosor promedio de 0,5 cm fijados con acetona y cortes de encéfalo de vacuno de igual grosor, fijados con formalina acuosa al 10%. La totalidad de las muestras son deshidratadas en acetona a -17 °C. Posteriormente se impregnan e incluyen en resina poliéster (P-4) a temperatura ambiente. Se obtuvieron muestras con una buena transparencia y solidez que permite ver en detalle muchas estructuras, similar a la obtenida con la técnica de plastinación en resina. Mediante un método relativamente fácil de realizar es posible obtener piezas anatómicas de muy buena calidad.


The polyester resin (P-4) has been used in several anatomical techniques, due to its low cost, easy handling and elicit. Among them, the inclusion, which is performed on samples leaving relatively opaque layers and an important thickness which does not allow a clear vision of the structures. There is also the plastination of anatomical slices with this polymer which although, technically sophisticated and complex has a high economic cost. The objective is to develop a method for anatomical section inclusion, improving the quality of classic technique. Distal segment of human lower limb anatomical slices were used with an average thickness of 0.5 cm fixed with acetone and bovine brain slices equal thickness, fixed with 10% aqueous formalin. All the samples are dried in acetone at -17 °C. Subsequently, they are impregnated and included on polyester resin (P-4) at room temperature. Samples were obtained with good transparency and solidity that allows observation of details of many structures, similar to that obtained with the resin plastination technique. Using this relatively easy method we can get very good quality anatomical samples.

ABSTRACT

O presente estudo tem o objetivo de apresentar um relato de caso sobre condroma extra-esquelético ou de partes moles, uma lesão rara e benigna diagnosticada através da clínica, radiologia e exame histopatológico. O paciente (24 anos) compareceu ao Setor de Ortopedia do Hospital Unimed Betim com queixa de aumento de volume no polegar esquerdo, indolor há aproximadamente um ano. Durante o exame físico verificou-se presença de uma lesão nodular, de consistência dura, móvel, indolor que não acarretava em alteração funcional do dedo. O paciente foi encaminhado para setor de radiologia do hospital onde foi realizado RX e posteriormente ultrasonografia e ressonância magnética do polegar esquerdo. Diante dos achados dos estudos de imagem a equipe optou pela biópsia excisional da lesão. O exame histopatológico confirmou o diagnóstico de condroma extra-esquelético. Os elementos definidores do condroma de partes moles são: 1) curso clinico benigno e sintomatologia frusta; 2) ausência de conexão entre o tumor e estruturas adjacentes como periosteo, capsula articular ou osso; 3) crescimento lento; 4) ausência de prevalência por sexo; 5) características radiológicas e histológicas do tumor. O diagnóstico raramente é aventado quando confrontamos tumores de partes moles das mãos. A excisão é simples e deve ser completa para se evitar recorrências. A avaliação histopatológica deve ser realizada para confirmação diagnóstica.


The present study presents a case report of extraskeletal chondroma or soft tissue chondroma, a rare and benign lesion, diagnosed by clinical findings, imaging studies and histopathological exam. The 24-year-old patient presented at the Orthopedics Section of Unimed Betim Hospital with a slow painless growth on the left thumb about 1 year ago. During the physical examination, a nodular lesion was noted; it had a hard consistency, was mobile and painless, and didn't cause functional impairment for the finger. The patient was referred to the imaging unit of the hospital, where an X-ray was performed; later on, ultrasound and magnetic resonance imaging of the left thumb was done. After the imaging studies, the medical team chose excisional biopsy of the lesion. The histopathological examination confirmed the diagnosis of extra-skeletal chondroma or soft-tissue chondroma. Soft tissue chondroma is usually not considered for a soft-tissue tumor of the hand. Excision is easy and must be complete if recurrences are to be avoided. Histopathological evaluation is required for the final diagnosis.
